Checklist for children enrolled in one or more Kaleidoscope courses
- Kaleidoscope Medical/Pickup form (DUE JUNE 20, 2010)
- Allergy form (IF APPLICABLE - DUE MAY 20, 2010)
- Authorization to administer medication (IF APPLICABLE - DUE MAY 20, 2010)
- Note: Separate form required for each medication
Checklist for children enrolled in one or more camp courses
If your child is enrolled in one of the more active courses that Kaleidoscope offers, you must submit the following forms. These courses are: 28, 51, 55, 75, 85, 97, 106, 130, 147 and 158.- Completed Health History form (DUE MAY 20, 2010)
- Copy of child's immunization records (DUE MAY 20, 2010)
- Copy of child's most recent physical exam (DUE MAY 20, 2010)
- Note: Physical exam must have taken place no more than two years before the start date of course.
- Allergy form (IF APPLICABLE - DUE MAY 20, 2010)
- Authorization to administer medication (IF APPLICABLE - DUE MAY 20, 2010)
- Note: Separate form required for each medication
- Kaleidoscope Medical/Pickup form (DUE JUNE 20, 2010)
